These pages explain the practice and its planned consultation paths. They do not accept patient contact details, health answers, appointment requests, payments or resident lists.
While you browse
Opening a page sends a normal page request to the website host. The practice-topic filter works in your browser; its text is not sent or saved. This website has no advertising pixels or visitor submission forms.
Questionnaire and appointments
The protected questionnaire and online appointment requests are not connected. A future questionnaire will open in a separate practice-controlled service. A request or completed questionnaire does not confirm an appointment.
Contact the practice
For clinic appointments, call 678-547-0000. For concierge house-call requests, call 678-522-2870 during calling hours of 1–7 pm. These are not appointment hours. Please keep medical details out of hosting emails.
